The Performance Tasks PDF includes all required grade-level Performance Tasks for Grade 5.

Performance Tasks are also separated by content area in individual PDFs for the purpose of sharing grade-level materials with teachers in the same grade, organizing materials, and for use with assistive technology or augmentative and alternative communication devices.

Note: Some Performance Tasks identify Teacher-Provided Resources that are necessary to administer one or more activities in a Performance Task. These resources are physical objects that will be used by the Trained Teacher Administering the Alternate (TEA) to administer an activity. Not all Performance Tasks include Teacher-Provided Resources. Please check the List of Materials included in each Performance Task PDF to determine whether or not Teacher-Provided Resources are required.

Resources are also separated by content area in individual PDFs for the purpose of sharing grade-level materials with teachers in the same grade, organizing materials, and for use with assistive technology or augmentative and alternative communication devices.

Student Score Worksheet – Grade 5 [PDF]

Student Score Worksheets must be printed and used to record each individual students’ ratings aligned to each core extension for each activity for Grade 5.

This working document is used by the Trained Teacher Administering the Alternate (TEA) to record each student's ratings any time a Performance Task is administered over the course of the school year. Safely store the Student Score Worksheets following the completion of each Performance Task activity. The TEAs must save their hardcopy Student Score Worksheets for future reference.

Once all Performance Tasks are administered, the TEAs must enter the student ratings into the Data Entry Interface (DEI) for scoring. The TEAs must submit ratings for each student through the DEI so that student responses may be scored for state reporting. Submission must be completed between March 27 and June 2, 2023. Only Trained TEAs may submit Student Score Worksheets though the DEI.
